Service Agreement

Your Service Agreement with Professional Practice is governed by the Scope of Service (SOS), Terms of Service (TOS), and Business Associate Agreement (BAA).  Together, these three elements make up the Service Agreement.  The Service Agreement defines both parties' rights and responsibilities under the agreement. 

You will receive a countersigned copy of the Professional Practice Service Agreement when your Provider Intake form and all associated onboarding information and documents are received by Professional Practice.

Where can I get another copy?

When Professional Practice sets up your account with Office Ally-Practice Mate, your first patient will be "John Doe".  John Doe is a test patient, and one of the documents connected to John Doe's account will be a copy of your service agreement.

How do I access it?

Login

Login to your Office Ally-Practice Mate account using your username and password at https://cms.officeally.com/practicemate 

Manage Patients

Click on the "Manage Patients" tab in Practice Mate.

"John Doe"

Search for and open the account for "John Doe".

Documents

Open the patient account for "John Doe" and click the "Documents" tab below the patient information.

Service Agreement

Click the "View" icon next to your Service Agreement document to open and view your service agreement.

Is there another way?

Yes - You can email billing@professional-practice.org to request that we resend your current Service Agreement to you.  Please be aware that time spent to send documentation that was previously sent to the provider's office will incur an additional fee per the "Special Projects / Corrections" hourly fee.
