Patient Statements

When you start treatment, we bill your insurance per the information on your Billing Information Intake forms.  We will send you a statement if your insurance processes your claim(s) and indicates that you have a balance. Most insurance payers take 20-45 days to process claims.

If you have no insurance or your insurance denies your claim(s), then we will send you an invoice for the balance due.

If your insurance or contact information changes, please contact us right away.
